Nurse Prescriber

Join our warm and patient-centred practice in the heart of our semi-rural community. Enjoy a fulfilling 3 day per week role, where you’ll be welcomed by a friendly team dedicated to providing exceptional care.

As part of our Long Terms Conditions team, you will be responsible for areas such as health promotion, chronic disease management and health prevention. You will be a nurse prescriber with experience within a primary care setting, have excellent clinical skills and be competent in pro-active management of patients with chronic diseases including Diabetes, Asthma, COPD, Hypertension and Cardiovascular disease.

General Practitioner Locum - 6 sessions (6 months fixed term)
