To make it easier for patients to access their health information online without having to contact their practice, GP Practices have turned on full online access for patients (unless they have individually decided to opt-out or any exceptions apply). This means when you log into your NHS app, you will be able to see your full record details such as consultations, medications, test results etc. from the switch on date of 04/10/2023. This is automatic and no action is needed from you or your GP practice to turn this access on.
Please note, if you joined the Practice after 04/10/2023, you will have access to information from your date of registration onwards.
Should you wish to access information before this date, you must have been registered with the practice for at least 3 months. After this timeframe, you can complete the above form and bring into the surgery with 2 forms of ID (one showing photo ID and one showing your address) or call the surgery on 01539 760760 option 1 to answer some questions about your medical records to prove your identity. We can then, in most cases, provide you online access to information from 31/03/2016 onwards.
If you would like to access information from before 31/03/2016, you must fill in a Subject Access Data Request and we can provide you with a paper copy of the information you need. This cannot be made available for you to view online.
