Annual Diabetic Review

What is it?

Individuals with diabetes should have a review every year. This review will enable your doctors to keep track of your overall health and well-being.

This review is different to your 3/6 monthly blood test. Book a 'BLOOD TEST ONLY AS REQUESTED BY GP' slot for this.

What to expect at your Annual Diabetic Review.

Please bring a urine sample in a white topped sample bottle to your appointment. You can collect one of these bottles from either surgery prior to your appointment date.

Your annual diabetes care nurse will:

  • Take your height and weight (to check if you are under or overweight)
  • Take your blood pressure
  • Check your feet for ulcers or infections and ask if you have lost any feeling in your feet.
  • Take blood samples testing for HbA1c, cholesterol, renal profile, liver function tests and full blood count.
  • Send off your urine sample to check your kidney function.

After this appointment, you will be booked a follow up with the nurse specialist to discuss your results.

DO BOOK IF:

You have received your annual invitation to book your diabetic review. This will include the tests for any other long-term condition you may have as well as diabetes so you only need the one appointment.

 

DO NOT BOOK IF:

 You do not have diabetes.

You only need your 3 monthly or 6 monthly blood test. Book a 'Blood Test Only As Requested By Gp' slot for this.
